The Information We Collect.
When you use our site, we receive and collect certain information. The information we receive and collect depends on what you do when you visit the Website.
Automatically Collected Information.
Some information is automatically received and sometimes collected from you when you visit the Website. This information may include some or all of the following items: the name of the domain and host from which you access the Internet, including the Internet protocol (IP) address of the computer you are using and the IP address of your Internet Service Provider; the type and version of Internet browser software you use and your operating system; the type and version of your media player(s); the date and time you access our site, the length of your stay and the specific pages, images, video or forms that you access while visiting the site; the Internet address of the Website from which you linked directly to our site, and if applicable, the search engine that referred you and any search strings or phrases that you entered into the search engine to find this site; and demographic information concerning the country of origin of your computer and the language(s) used by it. We use this information to monitor the usage of our site, assess its performance, ensure technological compatibility with your computer, and understand the relative importance of the information provided on our site. We may also use this data to conduct statistical analyses on visitors' usage patterns and other aggregated data.
Pixel Tags or Clear GIFs, also known as Web Beacons or Web Bugs, are transparent graphical images placed on a website. We may also occasionally use "web beacons" (also known as "clear gifs," "web bugs, " "1-pixel gifs," etc.) that allow us to collect information about your response to our e-mail communications, and for other purposes. Web beacons are tiny images, placed on a website or e-mail, that can tell us if you've gone to a particular area on our Website. For example, if you've given us permission to send you e-mails, we may send you an e-mail urging you to use a certain feature of the Website. If you do respond to that e-mail and use that feature, the web beacon will tell us that our e-mail communication with you has been successful. We do not collect any personal information with a web beacon, and do not link web beacons with any other personal information you've given us.
Since Web beacons are used in conjunction with persistent cookies (described above), if you set your browser to decline or deactivate cookies, Web beacons cannot function.
Information You Actively Submit.
When you voluntarily provide or request information, subscribe to a service or mailing list, respond to an online survey or otherwise actively enter data, such as your name, mailing address(es), telephone number(s), medications, and dosage times, you will know what data we collect through the site, because you actively submit it to us. You can elect not to provide information by not entering it when asked. When you provide such information, you are agreeing that we may use that information to provide the products and services that you have requested or for which you have registered and to contact you to learn about your experience and to improve our product and service offerings.
You also agree that we may use that information when aggregated with other data in any manner in which the data is not linked to an identified individual, and to share such information with others, in the promotion of our business to potential investors, business partners or customers, and in the conduct of research or product development.
Unless you also actively submit personal information such as your name or address, passively tracked data does not typically allow us to identify you personally. To the extent permitted by applicable law, we reserve the right to combine passively tracked data with personal information that you actively submit.
Personal Information about Children.
The Sun Health Medication Management Program Website is targeted primarily for use by adults. Accordingly we do not knowingly collect any personal information from children under the age of 13. However, we advise all visitors to our site under the age of 13 not to disclose or provide any User Information on our site. In the event that we discover that a child under the age of 13 has provided User Information to us, in accordance with the Children's Online Privacy Protect Act (see the Federal Trade Commission's website at http://www.ftc.gov/privacy/privacyinitiatives/childrens.html for more information about this Act), we will delete the child's User Information from our files to the extent technologically possible.
How We Use and Share User Information.
When you actively submit User Information through the Website, we and/or the third party vendors who make Materials, Features, products and the Services available on or through the Website) will use that information in one or more of the following ways:
- To process, complete or otherwise act upon or respond to your request or reason for submitting that information;
- To register and/or verify you in connection with a service or feature that you are attempting to access or obtain;
- To communicate with you about your request or reason for submitting that information;
- To provide additional information to you about our respective services that may interest you;
- To study and analyze the use of the information and features available on our Website;
We do not sell User Information to third parties. Except where we otherwise obtain your express permission, we (and/or the third party vendors who make Materials, Features, products and the Services available on or through the Website) share your User Information with third parties only under the limited circumstances stated below:
- Credit card authorization companies receive the credit card number and other personal identifying information only to verify the credit card numbers and process a transaction.
- User Information is subject to disclosure in response to judicial or other governmental subpoenas, warrants and court orders served on us in accordance with their terms, or as otherwise required by applicable law.
- User Information may be disclosed and transferred if our respective businesses are sold or offered for sale to another company or person(s), if a petition for relief under the United States Bankruptcy Laws is filed by or against any one of us, or if any one of us becomes subject to an order of appointment of a trustee or receiver.
- If you communicate with us via email, we will share your correspondence, including any User Information provided in the email, with employees, volunteers, representatives or agents most capable of addressing your correspondence. We will retain your communication until we have done our very best to provide you with a complete and satisfactory response and may subsequently retain your communication for our records.
- As described above, we will share your information with the third parties who operate or maintain the Website, and/or the third parties who make the "My Medication Profile" and "My Personal Pharmacist" features available through the Website.
In order to carry out certain business functions, such as e-mail delivery or marketing, or making products, features or services available through our Website (such as the My Medication Profile and My Personal Pharmacist features), we hire other companies to perform services on our behalf. We may disclose User Information that we collect about you to these companies to enable them to perform these services, but they are required to safeguard the User Information and are not authorized to use it for any purpose inconsistent with the purposes or uses described above.
Our use and protection of health information which you actively provide to us, or which is actively provided by someone else to us, may be protected under applicable federal and/or state laws. To the extent that we receive information that is protected by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), we treat that information in accordance with HIPAA. We also require the third party vendors who make features, content, services or products available on or through the Website to treat that information in accordance with HIPAA. We, or those third party vendors, might alternatively receive information from you that is otherwise subject to the Health Information Technology for Economic and Clinical Health Act, or "HITECH Act," and, if so required, that information will be treated in accordance with applicable law.

Access to and Managing Your User Information.
We believe it is important for you to be able to find out what User Information you have provided to us through our site. If you have provided us with User Information, you can contact us to request that we provide you with the User Information we have in our records about you. We reserve the right to limit the number of times such a request can be made and to charge you for responding to such requests if this process is misused or abused. You may contact us using the contact information provided below to inquire about your User Information, and to correct, amend or delete such information. We want the User Information we have on record about you to be as complete and accurate as possible. If you believe that any User Information we have in our records about you is inaccurate, incomplete or incompatible with the purposes for which it was provided or subsequently authorized by you, please notify us using the contact information provided below.
What You Need to Do to Protect Your Personally Identifiable Information.
You have several options when deciding how you can best protect your User Information. One option is simply not to volunteer it. As stated above, this approach would allow you to still visit our site – although it will prevent you, for example, from taking advantage of some of our site's features. Use of Electronic Mail.
We may provide email links on the Website to further facilitate communication between us and you. Information collected through email may be shared with our customer service group, employees or other third parties that perform services on our behalf. Unless otherwise noted, email on our site is not a completely secure and confidential means of communication. Non-encrypted email communications may be accessed and viewed by other Internet users without your knowledge and permission while in transit to us.
